This letter is to inform you of our intent to provide home instruction for our child in the upcoming 2002/2003 school year as provided for by section 22.1-254.1 of the Code of Virginia, in lieu of having him/her attend school. His/her name, date of birth and grade level are as follows:

We have attached to this notice a program of study for the coming year which includes the State Standards of Learning objectives for language arts and mathematics for our child. We feel that we can provide an adequate education for our child(ren) by exercising diligence in teaching our child(ren) in a responsible way and by utilizing an organized curriculum, namely, (name curriculum) to instruct our child(ren).


We understand that by August 1st of next year we must provide evidence of educational achievement as prescribed in Section 22.1-254.1 of the Code of Virginia which defines the requirements for home instruction.


Copies of my child's achievement test scores for this previous year are also attached, as in compliance with the requirements of the law.
